Assessing Your Needs

If you are an adult or carer with care and support needs then we may be able to provide you with assistance.

You may be eligible to have an assessment undertaken regarding your circumstances. This process will be undertaken by a trained individual, and will help the council decide if you are eligible for care and support and funding from the council to help pay towards your care and support needs.

Anyone is able to ask for an assessment to be undertaken. You may be referred by someone you know or a professional, such as a GP. Alternatively you can complete the on line forms by clicking below

Eligibility

National eligibility criteria for adults with care and support needs is based on identifying how a person's needs affect their ability to undertake day-to-day activities, and how this impacts on their wellbeing.

Not everyone who completes an assessment with their local council will be eligible to receive care and support services from them.

Under the Care Act 2014 you will have eligible needs if you meet all of the following criteria:

  • You have care and support needs as a result of a physical or mental condition or an illness
  • Because of those needs, you cannot achieve two or more of a set of outcomes set out in the Care Act 2014

 The outcomes include the things which you might expect to achieve in your day-to-day life such as:

  • Managing and maintaining nutrition
  • Maintaining personal hygiene
  • Managing toilet needs
  • Being appropriately clothed
  • Being able to make use of your home safely
  • Maintaining a habitable home environment
  • Developing and maintaining family or other personal relationships
  • Accessing and engaging in work, training, education or volunteering
  • Making use of necessary facilities or services in the local community
  • Carrying out any caring responsibilities the adult has for a child

If you are not eligible for support / funding from the council, there are external agencies within the Local Authority that will be able to provide certain services to provide care and support.
